Student Immunization & Medications

Dear Parents,

If your student has any health conditions, updated immunization records, and/or will be taking any medications during school this year, please stop by their school nurse's office to obtain necessary paperwork and ask any questions that you may have.

Open house is a great time to do this! There is paperwork that MUST be completed and signed by the student's physician if they have asthma, diabetes, seizures, and/or food allergies.

If they will be taking any medications during the school year, there is a form to be completed by the physician for that as well.
